The vehicle that Permission is requested for should belong to, or be in the possession of an Operator under any leasing, hiring, loan or other similar arrangement.
If Permission for a vehicle is granted the Operator must ensure that the Permission conditions are compiled with.
If an applicant leases or loans an exempted vehicle to another User the responsibility for compliance with the conditions of the Permission
remains with the applicant. If the applicant transfers responsibility of the vehicle, whether leased or loaned, that person will become
the User and must apply for the Permission.

Please indicate the estimated frequency of journeys essential for your vehicle(s) to make on restricted roads during the controlled hours.
Frequency of more than once a month would be considered as frequent.

Enter the full addresses of up to three destinations situated in the Scheme Area which it is essential for your vehicles to regularly visit during the controlled hours.
If you do not have regular destinations, please give three examples of recent destinations situated within the Scheme Area.
For each destination entered, state the time at which vehicle loading/unloading is necessary.
To comply with Permission Conditions 4 and 5 each vehicle journey must be planned so that maximum use is made of the M25 and/or excluded roads until the nearest practicable point to their destinations.
This ensures the minimum use of the restricted roads.
Please provide the exact routes your vehicle(s) will use to travel between the locations given in answer to Question 5(a) and the destination(s) given in Question 5 (b).
(NOTE: excluded roads are shown as dark green on the map and their use is not restricted by the London Lorry Control Scheme).
Permission needed ONLY for vehicles Over 18 Tonnes.
The maximum gross weight is indicated on the vehicle’s Department for Transport Plating Certificate.
If the vehicle is articulated or equipped with a drawbar, enter the gross train weight of the vehicle and trailer.
PERMISSION IS ONLY REQUIRED FOR VEHICLES IN EXCESS OF 18 TONNES MAXIMUM GROSS WEIGHT.
